WeTHRIVE!sm Hamilton County Healthy Kids Ohio Learning Collaborative is a primary care practice improvement collaborative created to help primary care practices test and implement proven concepts for improving obesity screening, assessment and management. Cincinnati Children's Hospital Medical Center is collaborative with Hamilton County Public Health to conduct this project. The Centers for Health Care Quality and Child Policy Research at Cincinnati Children’s Hospital Medical Center will be recruiting primary care practices in Hamilton County to participate in this collaborative beginning in early November, 2010. Collaborative activities will begin in January 2011. | ||
The aim of this collaborative is to help primary care practices establish systems to ensure that every child receives proper preventive care, evaluation, and treatment for pediatric obesity, and that each child is linked with needed community resources. Participating practices will receive the tools and coaching needed to assist them with implementing changes based on the needs of their practice. Parent surveys will be conducted at the beginning and end of this collaborative to determine how practice changes regarding nutrition and physical activity counseling affect the patient and parent experience. | ||
