 PROJECT OVERVIEW

The aim of this collaborative is to assist primary care practices in establishing systems to ensure that every child receives proper preventive care, evaluation, and treatment for pediatric overweight and obesity, and that each child is linked with appropriate community resources.

Participating practices will receive the tools and coaching needed to assist them with implementing changes based on the needs of their practice.
